IPSEN CARES ENROLLMENT
After you and your doctor initiate your enrollment into IPSEN CARES, you must sign the Patient Authorization Form to complete the enrollment.

BENEFITS VERIFICATION, PRIOR AUTHORIZATIONS (PAs) & APPEALS
We will review your insurance coverage with you and inform your doctor’s office on how to submit any required documents to your insurance company.

FINANCIAL ASSISTANCE
We determine if you are eligible*† and help you enroll in any of our financial assistance programs for which you may qualify, including:
- Copay support* for eligible commercially insured patients
- Patient Assistance Program† for those experiencing financial hardship

CONTINUED SUPPORT
We continue to support you during treatment, including answering your questions and providing information about your treatment.

Meet Your IPSEN CARES Team
PATIENT ACCESS MANAGERS (PAMs)
When you enroll in IPSEN CARES, you will be connected with a dedicated PAM.
PAMs are knowledgeable about health insurance and prescription coverage. Your PAM can help you:
- Review your specific healthcare coverage needs
- Help prepare you to speak with your doctor, specialty pharmacy, or insurance company
- Identify financial support programs for which you may qualify

PATIENT EDUCATION LIAISONS (PELs)
You will also be connected with a PEL, a healthcare educator who can provide you with additional education and support.
Your PEL can:
- Provide information and educational materials to help you, your family, and your caregivers better understand your condition and treatment expectations for Sohonos
- Help support your specific situation and healthcare needs in alignment with the treatment plan created by you and your doctor
- Support you and your caregivers through some of the many challenges of living with a chronic condition
IPSEN CARES PELs are provided by Ipsen and do not work under the direction of your healthcare provider or give medical advice.
